Climbing and fall-protection harnesses

Petzl America issued two separate recalls of professional fall-protection harnesses on May 14, both flagged for death-risk fall hazards:

Lead: Duloxetine nitrosamine cluster (Class II)

Three concurrent Class II recalls were initiated around April 21–29 for N-nitroso-duloxetine impurity above FDA's 0.83 ppm limit, identified at 12- and 18-month long-term stability intervals:

  • [D-0516-2026] Ajanta Pharma duloxetine DR capsules, 20 mg, NDC 27241-097-06.
  • [D-0522-2026] Breckenridge Pharmaceutical duloxetine DR capsules, 60 mg, NDC 51991-748-90 (manufactured by Towa Pharmaceutical Europe in Spain). The cluster suggests a broader nitrosamine surveillance issue across multiple duloxetine API sources.

Other drug recalls (all Class II unless noted)
